| 12/24/2014 | Order of conditions for three years and the physician must obtain a clinical competency assessment. The physician is not permitted to prescribe or dispense controlled substances, until she demonstrates a competency to do so and remediation of any deficiencies identified in the underlying investigation and/or assessment after which further conditions may be imposed. Effective October 21, 2015 the physician had completed her clinical competency assessment and she is subject to the following conditions which are in addition to those previously imposed: the physician may only practice in a group practice that consists of no fewer than three physicians other than herself for a minimum of three years; the requirement of a practice monitor will continue for three years and she must engage in continuing medical education for five years.
